[Title:[ Shahbaz **Responsible to: Staff Nurse Qualifications: High school diploma or its equivalent. Must have a valid and current Certified Nurse Aid certification. Must complete Green House-Training Program. Must be able to read and write and have basic communication skills. Must be willing to learn and to be taught by others. Must have a positive attitude and ability to work as a team with all staff members.

Physical Demands: Must have good physical and mental health, good sight and hearing. Must be able to turn, bend, stretch, and lift to assist residents, make beds, move equipment and perform other related tasks. Using proper techniques, must be able to transfer unassisted weight -bearing residents weighing up to 125 pounds from bed to chair to toilet. With the assistance of another person or Hoyer lift, must be able to transfer residents weighing up to 250 pounds.

Job Description: The Shahbaz role is viewed as a universal worker a blended care giver role in which the care giver will provide care to the elder in all aspects of his or her daily life. The Shahbaz will provide care to the elders in a Greenhouse home. Their primary role is to protect, sustain, and nurture the elders through providing assistance with activities of daily living and meeting other needs as required. The core goal of the Shahbazims work is to provide assistance in a manner that promotes a high quality of life and a positive elderhood. The responsibilities of the Shahbazim include care of the elders and the elders environment, including cooking, laundry and housekeeping.

  • Facial care, oral care, nail care, hair care
  • Bathing, dressing, grooming and shaving
  • Bowel and bladder care / perineal care

Duties:

Recognize and respond to the needs of the elders

Answer the phone, door, and greet visitors in a pleasant manner

Promote autonomy of elders in decision-making

Maintain confidentiality and privacy of elder care, procedures and documentation

Work with the Clinical Team to discuss elders needs, progress, or changes in condition

Complete electronic charting as required during your shift

Follow the plan of care

Promptly answer elder call lights

Engage with elders to provide meaningful activities

Serve nutritious meals per the posted menu and the stated preferences of the elders

Serve snacks in between meals and at bedtime

Keep all area of the home clean, performing routine housekeeping duties including laundry

Report accidents, incidents, and any suspected abuse as required

Report any unsafe conditions or items needing repair as required

Complete in-services as required
