Highlights
Community Health Network seeks a Nurse Practitioner or Physician Assistant to join our exceptional, compassionate palliative care team in the home setting. Ideal candidates will have demonstrated expertise in the advanced practice of nursing with chronically ill patients and have an understanding of the specific needs of assessing patients as they navigate complex health challenges. Ideal candidates will possess knowledge of local resources, have prior house call/home care experience, and be able to compassionately and effectively provide care as a part of multi-disciplinary teams, including palliative care and hospice services. Ideal candidates will also have the ability and desire to work independently and have genuine interest in delivering care in the home setting.
Licensure & Certification
Educational background consistent with requirements for licensure and certification within the state of Indiana.
